For years, GLP-1 weight-loss treatment has meant a weekly injection. That is now changing. The UK has approved its first daily GLP-1 tablet, and for the many people who find injections off-putting, it opens a needle-free door. Here is a clear, honest guide to what it is, what the evidence shows, and, crucially, how to take it so it actually works.
What is the Wegovy pill?
The Wegovy pill is oral semaglutide 25 mg: a once-daily tablet for weight management, containing the same active ingredient as the Wegovy injection. Semaglutide mimics a natural gut hormone, GLP-1, that acts on the parts of the brain regulating appetite. It helps you feel fuller for longer, reduces hunger and eases cravings. Used alongside a reduced-calorie diet and more activity, it supports weight loss. The headline difference is simple: no needle.
Is it approved, and can I get it?
Yes, it is approved. But approval and availability are not the same thing. The MHRA authorised it on 11 June 2026, making the UK the first country in Europe to license it, for adults with obesity (BMI 30 and above) or overweight (BMI 27 to 30) with a weight-related condition. It is available to Envigore patients from 6 July 2026. It is not yet on the NHS. That is a separate NICE decision, unlikely before 2027 or 2028.
How much weight could you lose?
Approval was based on the OASIS 4 phase 3 trial: 307 adults over 64 weeks, alongside diet and exercise.
Trial averages. Real-world results vary, and the medicine works with diet and activity, not instead of them.
How to take it, and why getting it exactly right is non-negotiable
The Wegovy pill only works if it is taken correctly. It is paired with an absorption enhancer called SNAC, which creates a small protected window in the stomach so the medicine can be absorbed there before it is broken down. That window needs an empty stomach and only a little water. If it is disturbed by food, by other drinks, or by too much water, the medicine is not absorbed and the pill simply will not work.- Empty stomach.
- No more than 120 ml of water.
- Absolutely nothing to eat, drink or take for 30 minutes afterwards.
The rules in full:
- On an empty stomach: take it first thing in the morning, on waking, before you have had anything to eat or drink.
- Water, 120 ml maximum: swallow the tablet whole with a sip of plain water, no more than 120 ml. Do not exceed 120 ml, because more water actually lowers how much is absorbed.
- Then nothing for 30 minutes: no food, no coffee, tea or juice, not even more water, and no other medicines or multivitamins, for at least 30 minutes afterwards.
| ✓ ALWAYS | ✗ NEVER (including for 30 minutes after) |
| Take it on an empty stomach, first thing on waking | Eat any food |
| Swallow it whole with a sip of plain water, 120 ml maximum | Drink coffee, tea, juice, or any more water |
| Take it at the same time each morning | Take other medicines or multivitamins |
| Wait the full 30 minutes before anything else passes your lips | Exceed 120 ml of water at any point |
| Step (minimum 1 month at each) | Once-daily dose |
| Starting dose | 1.5 mg |
| Step 2 | 4 mg |
| Step 3 | 9 mg |
| Maintenance dose | 25 mg |

If you miss a dose, skip it and take the next tablet the following morning. Do not take two in one day.
How does it compare to the injection and to Mounjaro?
At standard doses the pill is broadly comparable to the Wegovy injection. Against Mounjaro (tirzepatide), it is important to be straight. In head-to-head trials of the injections, tirzepatide produced greater average weight loss. There is no head-to-head trial of the pill against Mounjaro, so any comparison is across separate studies.
| Wegovy pill | Wegovy injection | Mounjaro | |
| Format | Daily tablet | Weekly injection | Weekly injection |
| Active ingredient | Semaglutide | Semaglutide | Tirzepatide |
| Trial weight loss | ~14%, up to 16.6% (64 wks) | ~17.5% at 2.4 mg (72 wks) | ~20%+ at higher doses |
| Needle-free? | Yes | No | No |
| Head-to-head vs the pill? | N/A | Same drug; broadly comparable | No head-to-head trial |
Figures come from separate trials and cannot be compared head-to-head. Which is right depends on your health and goals, and is a decision to make with a clinician.
Already on the Wegovy injection? Switching explained
Because the pill and injection share the same active ingredient, semaglutide is the only weight-loss GLP-1 with a published two-way switching protocol. If you are stable on the 2.4 mg weekly injection, the usual approach is to take your final injection as normal, wait seven days, then start the 25 mg tablet on your next scheduled injection day. There is no exact milligram-to-milligram match, so lower injection doses are mapped to the closest tablet strength. A weekly injection holds a steadier drug level than a daily tablet, so appetite can feel slightly less even for the first few weeks. Switch under clinical supervision, and you can switch back if needed.
What about side effects?
As with all GLP-1 medicines, the most common side effects are gastrointestinal: nausea, diarrhoea, constipation and vomiting. These are generally mild to moderate and settle over time, which is why the dose is increased slowly. Less common but more serious risks, such as pancreatitis, gallbladder problems and dehydration, are discussed at your consultation. It is not suitable for everyone, including in pregnancy or breastfeeding. It is prescription-only for good reason: a clinician needs to review your history first.

The bottom line
The Wegovy pill is a real milestone: the first time effective GLP-1 weight-loss treatment comes as a daily tablet. It works best with diet and activity, it has strict dosing rules that genuinely determine whether it works, and whether it is right for you is a clinical decision to make with proper support. If injections have been the thing holding you back, this changes the conversation.+ View References & Further Reading
