Why the dose you maintain on matters, and what we’ve built to help you stay there
There’s a quiet truth about medical weight loss that people don’t always talk about.
You spend months, sometimes years, working towards your goal weight. You finally get there and almost immediately realise the journey isn’t quite over.
If you’re reading this, you’ve either reached your goal weight or you’re beginning to think about what comes next. Either way, the question is the same.
How do you maintain your weight loss once you’ve reached your goal?
The honest answer, backed by clinical evidence, is that many people who stop GLP-1 medication without a structured maintenance plan regain a significant proportion of the weight they’ve lost. That’s not a scare tactic. It’s biology.
That’s exactly why we developed the Envigore Maintenance Programme, helping patients transition from active weight loss to long term weight management with the right level of clinical support.
What the clinical trials tell us
Two landmark clinical trials have explored what happens after patients reach their goal weight on GLP-1 medication.
The first, SURMOUNT-4, studied tirzepatide, the active ingredient in Mounjaro, in people who had already achieved significant weight loss during a 36 week treatment phase at their maximum tolerated dose of 10 mg or 15 mg. Participants were then randomly assigned to either continue taking tirzepatide or switch to a placebo for a further 88 weeks.
The results were clear. Those who stopped treatment regained, on average, around 14 percentage points of body weight. More than four in five participants regained at least a quarter of the weight they had originally lost. In contrast, those who continued on their maintenance dose were more likely to maintain their weight loss while also continuing to see improvements in key metabolic health markers.
The STEP 4 trial reported similar findings for semaglutide, the active ingredient in Wegovy. Participants who had already lost weight on semaglutide 2.4 mg were either kept on the same dose or switched to a placebo. Those who continued treatment maintained their weight loss and achieved further modest reductions, while those who stopped regained a meaningful amount of weight within months.
Together, these studies reinforce an important message. For many people, GLP-1 medication is not simply a short course of treatment with a fixed end point. It’s one part of a longer term weight management strategy, and choosing the right maintenance dose can play an important role in helping people maintain their results over time.
Why the maintenance dose matters
Not all doses offer the same level of evidence when it comes to long term weight management. The manufacturers’ clinical trials evaluated specific maintenance doses, and those strengths continue to provide the strongest evidence for helping people maintain their results over time.
For Mounjaro (tirzepatide), the SURMOUNT-4 trial evaluated the maximum tolerated doses of 10 mg and 15 mg during the maintenance phase. These strengths have the most robust long term data for helping patients maintain their weight loss while continuing to support improvements in metabolic health. They form the foundation of our Momentum programme.
For Wegovy (semaglutide), the STEP programme evaluated the 2.4 mg maintenance dose. This remains the dose with the strongest clinical evidence for supporting long term weight management and maintaining weight loss after reaching a goal weight.
That said, there is no one size fits all approach to weight loss. Every patient is different, and the right maintenance dose will depend on your individual health, treatment response and how well you tolerate the medication.
For some people, a lower maintenance dose may be the most appropriate option. Where clinically suitable, our prescribers may recommend Mounjaro 5 mg or Wegovy 1.7 mg to help maintain weight loss while balancing effectiveness and tolerability.
That’s why the Envigore Maintenance Programme offers Mounjaro at 5 mg, 10 mg and 15 mg, and Wegovy at 1.7 mg and 2.4 mg. These are the maintenance doses our clinical team considers appropriate for long term weight management, combining the available clinical evidence with personalised care.
Ultimately, the right maintenance dose isn’t chosen by a pricing table. It’s agreed between you and your prescriber, taking into account your progress, your goals and what will help you maintain your results in the long term.
What the Envigore Maintenance Programme offers
We have designed two pathways, depending on where you are in your journey.
Stride: Medication-free maintenance (£20 per month)
Stride is for patients who are ready to come off medication entirely. For £20 per month, it provides ongoing clinical check-ins, digital nutrition and exercise resources, a monthly newsletter, and access to our patient support forum. Stride keeps you connected to your clinical team, so if anything changes you are already within the care pathway and can step back into treatment without starting from scratch.
Momentum: Medication-supported maintenance
Momentum is for patients who need to stay on a maintenance dose of medication. It is a six-month, all-inclusive subscription that wraps your medication, clinical support and lifestyle guidance into a single preferential monthly price. Every Momentum patient receives:
- A personalised initial nutrition consultation with a qualified specialist dietitian at the start of your subscription. This is a 45-minute review of your current diet, health goals, lifestyle and nutrition challenges, with tailored guidance and clear next steps to support your maintenance.
- Prescriber dose reviews every four weeks at reorder.
- Direct access to our nurse-led WhatsApp group, Monday to Friday.
- Digital diet books and a monthly maintenance newsletter.
- A monthly supplement discount.
- A blood test discount, coming soon when the service launches.
Your medication can be bought one pen at a time, or in bundles of two or three pens of the same strength, whatever suits your routine. At the end of each six-month subscription, your prescriber reviews whether your dose is still right, because bodies change and medication should change with them.
If you need to cancel within the first three months
We have kept this as fair as possible. The only cost we ever ask you to cover is your dietitian consultation, and only if you have actually had it:
- If you cancel before your dietitian consultation has taken place, there is no charge at all.
- If you cancel before the end of your subscription period, a small fee will apply.
- Any medication already sent to you cannot be refunded, which is the case with all medicines. Anything not yet dispatched is simply cancelled, with nothing further to pay.
- And if you ever need to stop for medical reasons (confirmed by our prescriber), or your prescriber advises it, no fee applies. Your health always comes first.
Who can join?
To join Stride or Momentum, you’ll need to meet the following criteria:
- You’ve reached your personal goal weight.
- Your BMI is between 19 and 25 (with flexibility where clinically appropriate).
- You’ve been taking prescription weight loss medication for at least three months, either through Envigore or another provider.
If your current dose isn’t one of our recommended maintenance strengths, your prescriber will work with you to find the most appropriate option before you join.
Every application is reviewed individually by our clinical team because effective weight management is always personal.
